Rhinoplasty Types and Techniques

Nose surgery takes many forms and is one of the most complicated procedures a surgeon can perform. Additional skill and experience is required for a surgeon to be able to achieve a successful outcome for a wide range of patients, including those considering "ethnic" rhinoplasty or revision rhinoplasty. Rhinoplasty is a popular cosmetic procedure, and according the American Society of Plastic Surgeons, it was the second most common cosmetic procedure in 2009. There are several different forms that nose surgery can take, which makes the procedure all the more complicated. As a San Diego rhinoplasty surgeon, I believe that in order to achieve optimal results for every patient, the surgeon must be skilled in and familiar with the many different rhinoplasty techniques.

Closed vs. Open Technique

The closed and open techniques define the incision placement for rhinoplasty. To perform a closed rhinoplasty procedure, the incisions are created on the inside of the nose. When performing an open rhinoplasty, incisions are created inside the nose with an additional incision in the columella between the nostrils. Closed rhinoplasty leaves no visible scarring, but does not provide the degree of access of an open procedure. While open rhinoplasty requires an incision across the columella, the resulting scar is well hidden and becomes virtually imperceptible after a few weeks to months. Whether San Diego plastic surgeons use the closed or open technique will largely depend on the patient's goals for nose surgery.

Ethnic Rhinoplasty

There is no "ideal" nose, so it is important for a rhinoplasty surgeon to take into account the different aesthetics within different cultures as he or she tries to create improved balance and facial harmony through nose surgery.

African American: A common characteristic of the African American nose is a low nasal bridge and a wide tip. Typically, an African American patient will seek to have the nose augmented to provide better balance to the face.

Mediterranean: Most patients with Mediterranean nasal characteristics typically wish to reduce the hump at the bridge of the nose. In many cases I may recommend retaining a slight hump in order to retain ethnic identity while still achieving an enhanced look.

Asian: Oftentimes Asian patients wish to address a low bridge. By augmenting the bridge and improving the tip projection, patients can achieve a look that is both natural and beautiful.

Hispanic: Hispanic noses may be characterized by a wide bridge and an underprojected tip. Many Hispanic patients have surgical goals that are similar to Asian rhinoplasty patients.

With any ethnic rhinoplasty it is important to maintain the patient's ethnic heritage. Their heritage is part of what makes them unique and contributes to their overall beauty; therefore, it is often necessary not to alter the appearance of the nose too drastically.

Revision Rhinoplasty

Sometimes after having nose surgery, a patient is not happy with their new appearance. Whether this is because their plastic surgeon in San Diego delivered a "nose job" that does not look natural, because they are having breathing problems, or for other reasons, revision rhinoplasty can help improve the nasal appearance so that a person achieves their original goal.

Revision rhinoplasties are typically much more complex than the initial nose surgery due to the changes that have already occurred to the normal anatomy of the nose. Scar tissue may be present and the functionality of the nose may have been compromised, adding to the difficulty of the revision procedure. Depending on the patient's needs, implants or grafts can be used to rebuild the nose. Injectable fillers are also sometimes an option for small defects in the nose, though filler treatments will need to be repeated over time to maintain the appearance. The surgery will be dependent on each individual, as there are several different reasons why corrective nose surgery may be necessary.

No matter which techniques are employed, one of the most important ways to insure beautiful results is to find a surgeon who is skilled at nose surgery and has performed hundreds of rhinoplasty procedures.
