Liposuction using blunt catheters and negative pressure suction was first practiced in Europe in the late 70s of the past century. Three French surgeons were the first plastic surgeons to present their liposuction experience at the 1982 American Society of Plastic and Reconstructive Surgeons annual meeting in Honolulu, Hawaii. The procedure was initially faced with criticism in the United States. In late 1982, a “blue ribbon committee” was assigned by The American Society of Plastic and Reconstructive Surgeons to visit those pioneer plastic surgeons in Paris and the committee returned with a report that was cautiously optimistic. American surgeons’ interest in liposuction has steadily risen ever since, ahead of the public growing demand for minimally invasive body contouring procedures.

More individuals think that they need to undergo liposuction, although liposuction might not be the right procedure for their needs. Case selection is a crucial determinant of a perfect aesthetic surgical result, especially in body contouring procedures such as liposuction. Not all individuals who request liposuction are good candidates. A good consultation begins with an assessment of your goals. What do you wish to change about your body? What do you hope to accomplish with a liposuction? An experienced plastic surgeon would provide you with a realistic appraisal of what can and cannot be accomplished through liposuction.
Some cases may require alternative therapeutic procedures such as an abdominoplasty or a liposuction procedure that is combined with another open surgical procedure. An astute surgeon is wary of patients who are considered poor candidates for a successful liposuction procedure such as perfectionists with unreal perceived self deformities, severely depressed individuals who suffer from eating disorders and markedly obese individuals who are incapable of losing weight and expect liposuction to do it for them.
A thorough weight history is an essential part of any liposuction consultation. Ideal candidates are at a stable weight with a successful diet and exercise regimen going on. Patients who have a history of marked weight fluctuations are at high risk for weight gain after the procedure. Maintaining a stable weight and practicing a diet and exercise regimen for at least 6 to 12 months indicates the necessary commitment to lifestyle change.

Liposuction should not be resorted to as a treatment modality for obesity. In a perfect world, it is used to remove genetically distributed or diet resistant fat. In practical terms, however, it is often used to remove fat that could be lessened with diet and exercise. Ideal liposuction candidates are within 15 to 20 % of their ideal body weight or no more than 55 pounds above chart weight. Strangely distributed bulges of fat, or fat that is distributed outside the confines of the perfect body shape should be the target areas that are commonly suctioned.
