Get your baby's passport photos. The background must be white, so dress the baby in another color to prevent too much blending into the background. The baby must be awake with his eyes open, no pacifier and without his hands blocking the face. The head must take up 1 inch to 1 3/4 inches of the photo. The photo cannot be bigger than 2 by 2 inches. You need two identical photos.

Fill out form DS-11, which you can find on the State Department's travel website. Do not sign the application until you are at the passport agency.

Submit the application, the passport photos, certified copy of the birth certificate and copies of the parents' photo ID in person at the passport agency. Both parents must be present when submitting the application or one parent can be present with a notarized Statement of Consent from the other parent.
