Gastric Bypass More Effective than Lap-Band, Studies Show

The first study, conducted by Dr. Guilherme Campos at the University of California San Francisco, was published in the February 2011 issue of the Archives of Surgery. Gastric bypass surgery is more effective than the Lap-Band procedure in resolving Type 2 diabetes and prompting rapid weight loss, according to two recent studies.
The first study, conducted by Dr. Guilherme Campos at the University of California San Francisco, was published in the February 2011 issue of the Archives of Surgery. The study used 100 pairs of patients, equally matched in terms of age, weight, sex, body mass index (BMI), and severity of Type 2 diabetes symptoms. Each patient was randomly assigned either a laparoscopic Roux-en-Y gastric bypass procedure, or a laparoscopic banding procedure. The study then followed the participants’ lives during the procedure and for a year afterwards. Those patients who had the gastric bypass surgery lost twice the weight of those who had the Lap-Band procedure. They also had a higher degree of resolution for Type 2 diabetes—75% versus 50% of those who had the adjustable gastric band.
Dr. Campos feels strongly that the outcome of this study is a slam dunk for gastric bypass. In an interview for WebMD, Campos said, “It’s very clear—bypass is better than band, period. Bypass resolves more diabetes than band, period. And if you are diabetic with obesity, the best treatment is Roux-en-Y gastric bypass.” On the other hand, Campos feels that the Lap-Band may be the right option for some patients, especially obese patients with a lower BMI.
“Both the Lap-Band and the gastric bypass procedure have been shown to resolve diabetes and other co-morbidities,” noted Dr. David Provost, former Medical Director for the Surgical Management of Obesity at UT Southwestern Medical Center, who was not involved with the study. Research indicates that gastric bypass patients may have more success maintaining their weight loss long-term, he added, noting that the Lap-Band procedure has only been approved in the United States since 2001.
A second study, conducted in Taiwan, showed similar promising results. The study utilized 60 patients who had BMIs between 25 and 35 but also had difficulty controlling their Type 2 diabetes. Each pair of patients were evenly matched in terms of sex, age, weight, BMI, and severity of diabetes, and then randomly assigned either Lap-Band or gastric bypass surgery. Among the patients who had gastric bypass surgery, 93% saw resolution of Type 2 diabetes within the year, versus only 47% of those who had the adjustable gastric banding procedure. But the benefits didn’t stop there—gastric bypass patients also had lower LDL and total cholesterol levels, lower blood glucose, and greater amounts of weight loss.
While the data collected from both studies shows that gastric bypass surgery can be more useful in combatting some of the common health problems associated with obesity, the ultimate choice of which type of surgery is right still rests with the patient and his or her own surgeon. For the best results, patients are advised to do their homework and consult with an experienced bariatric surgeon before deciding which procedure best fits their needs.
