An evaluation form is used to assess the performance of an individual; it’s mainly used to measure the performance of an employee by his employer or a student by his teacher.

It’s a highly effective tool for company, administrators and teachers by the use of these forms strong points and weak spots of an individual can be easily determined. Properly designed evaluation forms provide plenty of authentic information by which valuable results can be obtained. The size of this form depends upon the kind of work and intricacy of the tasks.



Creating an Evaluation Form

Evaluation forms should contain various columns like employee’s name, section, date of appointment, review date, last review date and the name of the supervisor.
Use numerical rating for the review, for instance give number 1 for poor rating and number 5 for extraordinary.
Incorporate the categories for attendance, effort, discipline, initiative, work ethics etc. and provide space in each category where the rating will be given.
Also provide a skill column to mark rating in it about the talent that is specifically used in the company.
Use separate section for marking the managerial skills of the employee.
